學生申請時間： 即日起至 2026/10/8（星期四）下午 4:00

Student application period： From now until October 8, 2026 (4:00 pm)

配合校內成績排名作業時程及線上申請系統開放， 申請者一律須取得系排名相關成績後，再繳交紙本申請文件至醫環系辦。

To align with the on-campus ranking process and the online application system's opening, applicants must obtain the relevant departmental ranking scores before submitting the paper application documents to the Department of Medical and Environmental Sciences.

• 書面資料及線上申請資料文件缺一不可。
• 同學務必繳交符合本校規定之學業成績及語言檢定證書。
• 語言檢定成績須符合本校規定之有效日期： 2024年10月1日後考試之成績始符合規定。
  ※ 若為9月考試、10月才取得成績單，亦不符合規定。
• 若有刻意遞交或填寫不符資格文件之事實， 將視為不合格件，並列入本處列管名單中。 請同學務必遵守校方相關規定。

Important:

• Both the paper documents and online application materials are required.
• Applicants must submit academic records and language proficiency certificates that meet the University's requirements.
• Language test results must be from examinations taken on or after October 1, 2024. Results from an examination taken in September but issued in October are not accepted.
• Intentionally submitting or providing documents that do not meet the eligibility requirements will result in disqualification and may be placed on the relevant administrative watch list. Please strictly follow the University's regulations.
